Libby brings over 27 years of experience caring for older adults, including geriatric assessment, complex care management, and patient-centered clinical practice. In her role at ApogeeCare, she will lead interdisciplinary care teams to support frontline emergency clinicians and ensure that older patients receive care aligned with best practices in geriatric medicine.

In her new role, Libby will play a key role in strengthening clinical operations and deepening partnerships with hospitals, across Maryland and the Mid Atlantic, including Luminis Health AAMC. She will also help grow and support ApogeeCare’s clinical team while building meaningful relationships with health systems and community partners to advance shared goals and expand access to high quality geriatric care. Libby will also lead our clinical talent, recruitment and training to ensure that ApogeeCare’s clinical teams continue to deliver high-quality results for patients, their loved ones and our health system partners.

ApogeeCare’s model integrates telehealth-enabled specialty care with on-site clinical teams to improve patient outcomes, reduce unnecessary admissions, and optimize hospital capacity. At ApogeeCare, he will carry that mission forward, helping hospitals better serve complex, vulnerable populations, particularly older adults with multifaceted care needs.
