Lead Care Liaison (RN)
ApogeeCare is seeking a Lead Care Liaison (RN) to lead the Care Liaison team at a Houston-based emergency department. This role supports ApogeeCare's telehealth services by assisting remote clinicians during geriatric assessments, coordinating with ED staff and community partners, and overseeing training and daily operations of the liaison team. Requires an active RN license and at least 3 years of experience, with emergency medicine background preferred.
