A large regional hospital faced rising numbers of ED visits by older adults with complex care needs. The resulting ED crowding and prolonged hospitalizations proved challenging to navigate. Patients who needed inpatient care the most could be left waiting.
In early 2025, ApogeeCare launched with a dedicated care team providing consultative services to complex older adults. Patients received careful and comprehensive assessments soon after arrival to the ED to determine the full spectrum of an individual’s care needs. In collaboration with ED providers, ApogeeCare aimed to ensure timely and patient-centered care planning for the ED’s most vulnerable patients. The impact was immediate. One third of patients who seemed destined for admission were instead safely discharged or transitioned home after a brief observation stay. Nearly half of patients had unmet care needs that were uncovered and addressed by ApogeeCare, reducing the likelihood of returning to the ED. For those who did require admission, hospital stays were improved by early age-friendly intervention.
